The Aboriginal Health Worker employed at the hospital provides support and advocacy, links with health and other community based services, a follow up service after discharge from hospital, support and comfort when using hospital services, promotion of self management in using hospital services, assistance with the admission and the discharge process from the hospital, liaision between doctor and patient, referral to other providers, assistance with Centrelink forms and informal education to health workers: EntryProcedures The Aborignal Health Worker has good network/relationships with Murray Mallee Community Health Service, The Aboriginal Primary Health Care Unit, Aboriginal Aged Care Program, Kalaparrin Community and Aboriginal Legal Rights Movement and other hospital and health professionals
