Small team providing continuity of midwifery antenatal care and inpatient postnatal care for women and their families with identified vulnerabilities during pregnancy (eg domestic violence, mental health, substance use, young mums, homelessness). The complex care midwives work in collaboration with social workers, perinatal health, chemical use in pregnancy and non-government agencies to care and plan care.
