Community health services include home-based nursing, home and community care, lymphoedema clinic, asthma, diabetes education, lifestyle program, Telecross and Meals on Wheels. Day care at the hospital provides social, recreational activities and fitness for older people and people with disabilities (Tuesday and Thursday at 9.30am). Community mental health care for people aged 18 and over. Broad range of support services including triaged case management, grief and counselling
