Wimmera Hospice Care
The program provides consultative and supportive service for patients and their families.
The service loans equipment which is used to promote the comfort for the patient and to make care easier for the carer.
Coordinator available to be involved in educational sessions for professionals as well as community groups.
Referral may be made by self, medical, nursing or hospital staff. Patient's GP needs to be aware of the referral.
