Additional information
Hospital to Home provides a time limited short term intervention for four weeks post discharge which is designed to assist patients to recuperate following an acute hospital admission and to facilitate their independence, or transition to continuing care when required.
This care may include, home help, personal care, delivered meals, respite care and home nursing.
The program provides services to clients who meet the Home and Community Care criteria and are not covered by other programs.
