The Dementia Access and Support officers work with people living with dementia and or memory loss and their families to facilitate access to a wide range of services, organisations and supports appropriate to the client and their specific situation and need.
This service is available in Bass Coast, South Gippsland, Baw Baw, East Gippsland and Wellington LGA's.
