'A chronic disease management service for people over the age of 16 years with congestive heart failure, coronary artery disease, chronic obstructive pulmonary disease or diabetes who have difficulty managing their condition and are at risk of hospitalisation. Care co-ordination of these patients can be provided by Local Health District Care Co-ordinators. Further information can be obtained or referral by calling.
