•Home Visiting
- assessment for home safety
- modifications eg to home or car
- equipment prescription
- community access eg shopping, public transport
• Advocacy and assistance in accessing services
• Information
• Referral to other agencies
• Group sessions
- Planned Activity Groups, Paediatric motor skills group
• Health education ie back care, falls prevention
