We provide assessment and management of acutely unwell/deteriorating older persons residing in residential aged care facility where hospital transfer is being considered or further deterioration is possible. It is a 2 to 4 hour response.
Our aged care specialist team consists of: Geriatrician; Nurse Practitioner/Transitional Nurse Practitioner; Clinical Nurse Consultant.
