GPs can download the templates directly into their software. Templates are currently available for Medical Director and Best Practice using Medical Objects or Healthlink. Templates for other software packages are expected to be available soon. The template is updated quarterly and is available at http://www.gpqld.com.au/eHealth/eReferrals

Required information includes, Patient Demographics and contact person Medicare information (including expiry date) Reason for referral Specialist / Specialty referred to Relevant Clinical/Social & Family history Relevant tests/Investigation results Referring Practitioner and/or Usual GP (if different)
