The Geriatric Outpatient Therapy Service is an outpatient geriatric service for early diagnosis and intervention for falls, frailty, poly pharmacy, mobility and cognitive impairment.
The service includes the Active and Able 10-week multi-disciplinary team (MDT) program. The MDT consists of geriatricians, clinical nurse consultants, nursing, occupational therapy, physiotherapy and outreach dietetics.
