The ICCAP clinics are nurse led clinics to improve access for Aboriginal people aged 15 years and over who are at risk of diabetes, cardiac, respiratory and renal disease. The nurse led clinics consist of health screening and awareness; full comprehensive assessment comprising of Point of care testing; medication review; psychological and social assessment; health coaching; smoking cessation; referral to community services; GP's and specialists; action plans; follow up and support; in home suppo
