'The Aboriginal Health Practitioner's role aims to increase access to available health services for Aboriginal and Torres Strait Islander people in the community. It also increases the awareness of preventative health among the Aboriginal and Torres Strait Islander communities focusing particular attention on immunization, diabetes, alcohol and other drug abuse, sexual health, men's health, women's health and mental health. Aboriginal Health Workers are involved in and conduct many Health Promotional courses throughout the year for the Aboriginal community. Aboriginal Health Workers conduct a 'Gunya' program, of follow up (post discharge) and health visits to Aboriginal People with Chronic illnesses in the client's home. The aim is to increase the client's knowledge about their illnesses and prevention of unnecessary readmissions to hospital. '
