This program is a community based nursing led program that provides ongoing education, monitoring and support for adults living with Chronic Obstructive Pulmonary Disease (COPD) or other chronic respiratory diseases. Nurses assist people and their carers develop skills in self management. The service works closely with GPs and other health providers to promote best outcomes for those affected by chronic respiratory disease.
