ACT Health Community Care Nutrition Service offer adult dietary assessments and advice for nutrition related needs that are associated with medical conditions, weight management intervention, Home Enteral Nutrition Service (HENS), and a cardiovascular nutrition proram called Heart Fare Program.
ACT Health provides a free, community-based nutrition service in Phillip, Tuggeranong, City, Belconnen and Gungahlin Health Centres.
The nutrition service offers dietary assessments, weight management in group sessions, advice or counselling for nutrition related needs that are associated with medical conditions such as: minor gastro intestinal conditions, malnutrition, chronic renal disease, heart conditions, palliative care, swallowing issues and wound healing.
The nutrition service also manages the Home Enteral Nutrition Service (HENS) which can provide assessment, prescription and advice to patients, including advice on equipment issues.
Access into the nutrition service is via the Community Health Intake (CHI) line, by calling 5124 9977. Clients must be ACT residents 25 years and over (except for Enteral fed patients who must be 18 and over). Home visits may be offered to home/ bed bound patients only.

ACT Health provides a free, community-based Adult Healthy Weight Program in Phillip, Tuggeranong, City, Belconnen and Gungahlin Health Centres.
The program is run as a group session and it promotes a non-dieting approach for managing extra weight. Practical help is provided on changing your diet, being aware of your hunger and being more active. A dietitian, social worker and physiotherapist will assist throughout the program to help clients make changes.
The program is held at different times of the year and attendance involves 4 x 2 hour weekly sessions and on-going support. On-going support is offered through monthly appointments, a telephone call back system or via email.
Access to the Adult Healthy Weight Program is via the Community Health Intake (CHI) line, by calling 5124 9977. Clients must be ACT residents 25 years and over.

ACT Health provides school-based immunisation services as well as immunisation clinics for babies, toddlers and young people under the age of 18 years. Immunisation clinics operate out of different health centre locations across Canberra.

All the vaccines that are available on the National Immunisation Program (NIP) are given at the age appropriate stages. Free vaccines are available to children aged 0 - 5 years.
Children must either be accompanied by parent /guardian or have written consent from parent/guardian and each child must be at least 6 weeks of age to receive the first vaccine dose.
Self referral can be made through the Community Health Intake (CHI) line during 8:00am - 5:00pm weekdays by calling 5124 9977
Children who are in year seven or year nine within a Canberra high school are eligible for age appropriate vaccinations. ACT Health has a School Health Immunisation team which provides these vaccinations at your child's school at a prearranged date and time.
Information about vaccinations is sent home to parents/carers via the child's school. Consent is obtained at the beginning of the school year; however it may be some months before the vaccines are given.
Parents/carers can inform us of changes or withdraw their consent at any time. For more information on school based vaccinations phone the School Health Administrator on 6205 2086.

ACT Health Community Care Podiatry Services offer general podiatry assessments and treatment, biomechanical assessments, orthotic therapy, ingrown toenail treatment (including nail surgery) and the Footsure Health Promotion Program.
Eligibility: ACT residents with Centrelink concession card. Free service. Costs may be incurred for some items or supplies.
ACT Health provides a free, community-based podiatry service in Phillip, Tuggeranong, Dickson, Belconnen and Gungahlin Health Centres.
The service provides general podiatry assessment and treatment, biomechanical assessment and orthotic therapy, ingrown toenail treatment, including nail surgery. Eligible patients will be assessed for pedal risk. Those with high risk feet will be offered an ongoing treatment plan.
For patients with low risk feet and are unable to manage footcare (either self of carer) a referral to the basic Footcare Service is considered.
Access into the podiatry service is via the Community Health Intake (CHI) line, by calling 5124 9977. Clients must be ACT residents, and must hold a Centrelink Pensioner Concession card or a Health Care Card. Note: Seniors Card is not equivalent.

ACT Health offer management and advice on pelvic floor rehabilitation, bedwetting clinics, continence physiotherapy, bladder problems, faecal incontinence, continence services for men, bladder and bowel problems in children, self catheterisation and advise on continence products and aids. This service is free for ACT residents.
The Continence Promotion Centre is located at Level 3, 56 Lathlain Street in Belconnen (Belconnen Community Health Centre) and it provides management and advise on: – Pelvic Floor Rehabilitation, – Bladder Problems, – Faecal Incontinence, – Continence Services for men, – Bladder and bowel problems in children, – Self Catheterisation – Continence advice and assistance with products or continence aids. This service is free for ACT residents, however there is a fee for continence aids and appliances. Poor bladder and bowel control is a widespread health concern which affects around four million men and women in Australia of all ages and backgrounds.

ACT Pathology provides specialist pathology services to the general community, and pathology testing while patients are in hospital and when they return to their homes. We can bulk-bill patients with a valid Medicare Card directly to Medicare Australia. For the convenience of our patients, ACT Pathology has eight licensed collection centres throughout Canberra. These centres are staffed by experienced collection staff who have been extensively trained in blood collection techniques for adults, newborns and children.
When attending the Belconnen Community Health Centre collection centre, patients must bring a pathology request form from their treating practitioner. Patients should also bring their Medicare card to enable bulk-billing. The Belconnen collection centre operates on a drop-in basis, however tests such as Glucose Tolerance Test (GTT), Insulin Resistance Test / HOMA Index, Sweat test (only performed at Canberra Hospital) or ECG Service do require an appointment made prior. Our staff will answer your calls from 8am – 5pm Monday to Friday (excluding Public Holidays).
This service is available to residents of the ACT. Howver some of our services have eligibility criteria. You can access the service by ringing 5124 9977 and one of our staff will be happy to help you.
Our client service representative will register your details and ask specific questions about your health care needs. You may be transferred to one of our nurses for further assessment in order to determine eligibility and arrange the most appropriate service.

The role of the Adult Community Mental Health Teams located in Belconnen is the provision of specialist mental health assessment and treatment services by multi-disciplinary teams of medical and non- medical clinical staff. Treatment approaches are based on best available clinical evidence with an emphasis on positive outcomes that are tailored to an individual consumer's needs.
High risk assessments are managed taking into account the specific circumstances and the response indicated. Consultation services to General Practitioners are provided at each Team. On receipt of the GP referral for consultation, an appointment is made. For referrals for ongoing care with low to moderate risk, an assessment will be made by a mental health clinician. The assessment will be considered by the multi-disciplinary team who will communicate with the General Practitioner on the outcome. Liaison with community agencies as well as General Practitioners will occur.
The Adult Community Mental Health Teams primarily provide services to the residents of their local area. Under the principles of the 'no wrong door' approach, teams will respond to all presentations and facilitate assessment and referral to the appropriate area or service. The Adult Community Teams provide information and education on mental illness including treatment options and co-ordination of care with community agencies. Recovery principles guide the working relationship with consumers, carers and other stakeholders.

Footsure is a group health promotion program aimed particularly for those who have chronic illness, are elderly or disabled. Participants will learn how to identify common foot problems, manage nail and skin care, manage foot pain, learn how to choose a 'good' shoe and identify when to see a Podiatrist.
The session also includes practical demonstrations of footwear and foot products. A short DVD will be played and handouts will be available on foot care. The presentation aims to provide the knowledge, resources and skills to enable participants to manage their foot health with confidence.
The Footsure Program is only delivered in the ACT. It is a group session held on different dates within various health centre locations across the year. There is no eligibility criteria for clients, however access is via the Community Health Intake (CHI) line, by calling 5124 9977.

ACT Health offers free child health checks at various Child Health Clinics across Canberra. Child health checks are facilitated by Maternal & Child Health (MACH) nurses and can provide an opportunity for you to gain information, support and advice relevant to caring for your child.
Regular health checks are recommended to monitor how your child is growing and to offer early intervention and treatment if required for certain conditions. Your MACH nurse can check your child's development and growth and will discuss any concerns you have about your child.
Child health checks should be attended at about:
- 1 to 4 weeks,
- 6 to 8 weeks,
- 6 to 9 months,

ACT Health offers social work services for parents and families experiencing emotional, social and psychological difficulties that may have an impact on the health and well-being of the children and family.
ACT Health provide a range of accessible allied health and support services for babies, toddlers, children, young people and women. These services operate out of different health centre locations across Canberra.
Social work services are available to families in the antenatal and postnatal period and/or with children under school age.
Services include:
- Individual counselling;
- Therapeutic interventions with parents;
- Information, advocacy and support and
- Referral to and liaison with other support agencies.
Appointments are offered at Belconnen, Gungahlin and Tuggeranong Community Health Centres. Home visits are possible by arrangement between client and the social worker.

This service provides a full hearing assessment looking at the client's health history, otoscopy, tympanometry and audiometry.

ACT Health provides a free, community-based Heart Fare Program in Phillip, City, Belconnen and Gungahlin Health Centres.
The program is run as a group session and it covers healthy eating for clients with cardiovascular disease. Heart Fare will provide clients with up-to-date information on ways to reduce the risk of heart attack and stroke. The program covers issues such as healthy eating, dietary fats, maintaining a healthy body weight, label reading and recipe modification.
Heart Fare involves attending one 2 1/2 hour informative education session. Sessions in 2014 will be held monthly. Access into Heart Fare is via the Community Health Intake (CHI) line, by calling 5124 9977. Clients must be ACT residents 25 years and over.

ACT Health offers a Community Nutrition Service at Belconnen, Gungahlin, Tuggeranong, Phillip and City Health Centres and West Belconnen Child & Family Centre.
Eligible clients can access clinic based nutrition assessments, counselling and advice. Group education programs are also available, including Coping with Fussy Eaters, a group available to parents of children aged 1-5 years with fussy eating habits.
The nutrition service also offers health education, health promotion, nutrition consultancy and nutrition education resources for ACT residents. To be eligible clients must be:

- Children;
- Young people aged under 25yrs;
- Pregnant women, or
- Breastfeeding women.

CRT services are provided to adults with neurological conditions, including recent traumatic and acquired brain injury, spinal cord injury and chronic progressive neurological conditions. The CRT team provide comprehensive assessments and treatment to clients which may include physiotherapy, exercise physiology, occupational therapy, social work, speech pathology, counselling, psychology and vocational rehabilitation.
The Community Rehabilitation Team (CRT) is primarily located at Phillip Health Centre, however services are also provided within other community locations such as the client's own home, the Phillip Community Health Centre and Belconnen Community Health Centre.
The Community Rehabilitation Team (CRT) employs a team approach in the delivery of services and aims to improve client's functional capacity, retrain lost or impaired skills and support psychosocial functioning.
The CRT multidisciplinary team comprises of physiotherapy, occupational therapy, social work, speech pathology counselling and psychology, nurse practitioners, rehabilitation coordination, vocational assessment and rehabilitation.
CRT services are provided to adults with neurological conditions, including recent traumatic and acquired brain injury, spinal cord injury 18 months post discharge from hospital and chronic progressive neurological conditions requiring a multidisciplinary package of treatment.
The CRT team also provides services to clients with upper or lower limb amputations. In delivery of services the CRT maintains working links with Driving Assessment and Rehabilitation Service (DARS), Rehabilitation Medicine, Exercise Physiology and the Prosthetics and Orthotics Services (P&O) which are all services within the Division of Rehabilitation, Aged and Community Care (RACC).

Emergency dental services are available Monday to Friday (except public holidays) to all eligible clients of the dental health program who require urgent dental treatment. For urgent dental treatment, you can phone the clinic first, saving time and possibly an unnecessary trip. Limited after hour's emergency dental treatment is available at the Phillip Dental Clinic on Saturday mornings for Adults, Youth & Children.

ACT Health Community Care Physiotherapy Services offer community based physiotherapy for both adults and children with a range of movement problems, including musculoskeletal injuries, physiotherapy following orthopaedic surgery and/or fractures/dislocations, arthritis (in adults), connective tissue disorders, mobility issues related to acute injury, degenerative diseases, respiratory problems. The physiotherapy service aims to assist ACT residents to maintain independence and optimise function in the community.
ACT Health provides a free, community-based physiotherapy service in Phillip, Tuggeranong, Dickson, Belconnen and Gungahlin Health Centres.
The physiotherapy service is available for both adults and children with a range of problems including:
- Musculoskeletal pain, soft tissue injury, post treatment fractures, dislocations and post orthopaedic surgery.
- Acquired musculoskeletal pathologies, arthritis (in adults), connective tissue disorders, complex regional pain syndrome
- Mobility issues related to acute injury, degenerative diseases,
- Respiratory problems in adults and children.
Assessment and treatment is provided at clinics, and there is a limited home visiting domiciliary service available for patients who are physically/ medically home bound.
The physiotherapy service has a focus on patient education and self-management strategies.

The Asthma Nurse Educator Service provides children, young people, families and community groups with asthma education and support. Registered Nurses are able to provide education in the home to help clients understand asthma as well as the medications and devices used in controlling symptoms. The service works closely with GPs, paediatricians, specialists, hospital staff and the ACT Asthma Foundation. This service also provides education to registered nurses, student nurses, CIT students and other health professionals such as general practitioners and medical students.
This service is for the parents of young children, young adults (under 25 years of age) and community groups. The service is located at the City Health Centre, 1 Moore Street, Civic or alternatively the Asthma Educator's will arrange to see families in their homes, workplace, hospital or within a local health centre.
Any person under 25 years of age is eligible to use the service if a doctor has diagnosed them with asthma. Any individual seeking information about asthma is also welcome to contact the service.

ACT Health offer social work and support for clients who may be adjusting to recent medical diagnosis, discharge from hospital, living with chronic health conditions, or grieving the loss of a spouse or family member due to a medical condition. Multi-disciplinary wellness group work services are also offered throughout the year. Services are delivered from community health centres.
Social Workers work collaboratively with clients, their families, health care professionals and community organisations to contribute to the client and family's psychosocial health and well-being.
Social Workers are skilled professionals who can provide a comprehensive range of strategies, including counselling, advocacy, practical or emotional support, education and information.
ACT Health provides free social work for clients at the Tuggeranong and Belconnen Health Centres.
The social work team can offer:
- Support for adjustment to recent diagnosis, significant health condition, including chronic health conditions.
- Support regarding enhancing client's self care in the community and support for their spouse/carers.
- Community support that may assist in the prevention of social admissions into hospital including support for their spouse/ carers.
- Grief and loss support with respect to changing health status, health related bereavement and death of a spouse in the elderly.
- Counselling support for health related reactive depression.

The Community Care Program provides multidisciplinary services in the community setting. Services are available to residents of the ACT. Eligibility criteria apply and information can be obtained through the Community Health Intake.
Community Nursing:
- Community based clinical nursing services including wound, catheter and drain management, palliative care, medication administration and education.
- Specialist wound, stoma, and catheter services.
- Living a Healthy Life with Chronic Conditions courses available.
- Services are usually delivered in health centre clinics in the first instance unless Community Nursing deem that a home visit is required.
- Ambulatory clinics operate Monday to Friday in all health centres except Dickson with limited services operating from some Community Health Centres at weekends, bookings required.
- Link Team provides after hours community nursing services, including palliative care to patients overnight.
Continence Service:
- Continence services are available to clients who are experiencing bladder, bowel and/or pelvic floor dysfunction. This is not an acute service. Continence services are provided by Community Care Physiotherapy and Community Nursing and the Community Health Intake (CHI) can assist with directing referrals to the most appropriate service. The service includes the assessment, advice and management of most types of bladder and bowel dysfunctions and incontinence, pelvic floor dysfunction, pelvic pain, provision of advice regarding funding options for continence aids, and advice regarding a variety of continence aids and appliances in adults.

Services include: ACT Health offers monthly orthoptist clinics in each of the three locations:
- Belconnen Community Health Centre (Fridays),
- Tuggeranong Child and Family Centre (Fridays) and
- Gungahlin Child and Family Centre (Mondays).
This is a secondary vision screening service for children from birth to 6 years, if there are issues. Children access this service through a referral from a Maternal and Child Health (MACH) early childhood clinic or from school kindergarten screening.
The orthoptist performs: cover tests at 1/3 metre and 6 metres, assessment of ocular movements, tests for the presence of binocular vision, tests distance (6 metres) and near 1/3 metre visual acuity. If a child requires further assessment the orthoptist will refer on.
