Additional Information: Community Aged Care Packages (CACPs) provide client focused services to people requiring a low level of care, helping them to remain independent in their own home. CACPs provide a coordinated, planned and reliable service that enables clients to maintain control over services provided. Wesley House Aged Care Facility manages 39 packages. Indigenous people are given priority for 15 packages. Additional Information: The Project supports the employment, education/training and social participation of jobless families in the community by increasing the capability of at least one individual in the family.: EntryProcedures This program assists families and individuals, Aboriginal and Torres Strait Islander People, disability and mental health, homeless residing in Port Adelaide Enfield Local Council Area. Any agency can identify vulnerable families and refer them to the program.

Additional Information: Employment Access Port Adelaide provides professional guidance and support to people with disabilities, particularly mental health related disabilities, to find and maintain employment. Jobseekers work one to one or in a group with an Employment and Training Officer to develop a comprehensive Employment Pathway Plan to meet employment goals and needs. Jobseekers with barriers to employment are assessed by Centrelink using a Job Capacity Assessment.
