Additional Information: The Chronic Disease Telephone Coaching Service (CDTC) is a telephone based support service provided free of charge. The program will help people with chronic diseases improve their ability to self manage their condition. This leads to improved quality of life and reducing the incidence of exacerbation and progression of their disease. The Telephone Coaching program includes an individualised plan to help patients achieve their personal health goals, regular support calls from the program's registered nurse, advice on ways to improve aspects of the patient's health including weight management, understanding their medication and the importance of exercise, advice on effectively communicating with their doctor and access to 24 hour, 7 day a week telephone support. a referral from any treating clinician in required for Telephone Coaching

Additional Information: The Home Telemonitoring (HTM) service provides long distance telemonitoring of patients with a chronic disease. The Home Telemonitoring service provides installation of a telemonitoring station in a patient's home for anywhere between 1 and 3 months, patient education in the use of the home telemonitor to record their temperature, blood pressure, heart rate, heart rhythm, weight, oxygen saturation, blood glucose and respiratory function each day and monitoring of daily observations via the internet (Monday to Friday) by a clinician. The Home Telemonitoring service is currently accepting referrals via Community Health Intake (phone 6207 9977 or fax 6205 2611) from ACT Health Directorate Doctors, Allied health staff, Nurse Practitioners, and Clinical Nurse Coordinators.
