The Stroke Carers Group is a multidisciplinary education program for carers of people who have had a stroke. The program is aimed at assisting carers in their caring role.
The program is located in the Peter Heinz Centre, Queen Elizabeth Centre, Ascot St South, Ballarat with 4 - 6 weeks duration and is run intermittently when appropriate numbers are available.

Home and Community Care (HACC) Domiciliary Services provide a home-based occupational therapy service and home-based physiotherapy service for HACC clients who are unable to attend outpatient therapy.
Occupational Therapy service may include:
• Assessment of client's ability to manage activities of daily living that impact on safely living at home (ie. Bathing, toileting, transfers, kitchen tasks).
• Prescription of complex aids and equipment (ie, shower chairs, wheelchairs)
• Home assessment to determine the need for home modifications.
Physiotherapy service may include:
• Assessment, advice and education for clients and carers
• Assessment of mobility issues around the home or after a fall.
• Short term intervention and goal specific treatment

This is a not for profit, community based, outreach support and education service. The Grampians Service is based at Ballarat Base Hospital and is supported by Ballarat Health Services.
The counsellors are experienced social workers. Parent volunteers are experienced and trained parents who have been bereaved for more than two years. The worker visits families in their homes and offer ongoing bereavement support – as much or as little as the family requires. Support is provided to: parents, brothers and sisters, grand-parents, other family members, family friends, workmates and other members of the community.
Support, debriefing and education about grief and trauma is also offered to health providers, emergency service personnel, medical and allied health professionals. and the general community. Access is available to a library and other resources..
People may access the service themselves or by referral from health professionals, relatives or emergency personnel.
An office is based in the Edward Wilson Wing, Base Hospital, Drummond Sreet, Ballarat. Appointments at the office are preferred as the counsellor is often out on the road. If unavailable, there is an answering machine and a mobile number.
Support is also available from the Melbourne office which can also be contacted 24 hours a day on 1300 308 307, if the need is urgent and the counsellor cannot be contacted.

The Diabetes Prevention Program is a collaborative project with Ballarat Health Services in partnership with health, recreation, education and government.
The project aims to develop an effective and sustainable model to prevent or delay the onset of Diabetes Mellitus (Type 2) through a structured program over 16 months.
Target Group:
The project involves 120 particpants who may be at risk of developing Diabetes Type 2.
Inclusion Criteria:
-- People with impaired glucose intolerance or impaired fasting glucose
-- People who are 45 years and over who have risk factors of obesity (BMI >30) or hypertension
-- People with clinical cardioascular disease (myocardial infarction, angina or stroke)
-- Women with polycystic ovary syndrome who are obese
-- Women with previous gestational diabetes
-- People aged 45 years and over who have a first degree relative with Type 2 Diabetes
-- Considered appropriate by their General Practitioner for participation in a moderate intensity physical activity programM/
-- Willing and able to participate in all aspects of the program
Exclusion criteria:
-- People who have a medically unstable condition
-- People with uncorrected visual or hearing impairment
-- People who are intellectually disabled
-- People with diagnosed cognitive impairments (ABI, Dementia, Alzheimer's Disease and other conditions which impact upon information processing and memory)
-- People unable to participate in all aspects of the program
Phase 1 of the intervention commences with a six session Self Management Empowerment program.
Phase 2 includes a 12 week exercise program in which participants are randomised to either a Centre-based program or a Home-based 'Lifestyle' program.
The first two phases are intensive in nature and are followed by a Maintenance Program (Phase 3) This phase consists of four cycles of a motivational support group followed six weeks later by a newsletter.
Monitoring occurs at commencement and at relevant stages throughout the program.

The Paediatric and Adolescent Unit is a large and spacious ward consisting of eight different rooms/bays including a separate adolescent area of 3 beds. The Paediatric Unit admits both elective and emergency admissions, surgical and medical patients from new born to 18 years of age.
The ward has a play room for the toddlers and younger children. Other entertainment facilities include Nintendo, Playstation, as well as televisions over each bed and a video player in each room.
Adolescents also have a room that includes a stereo, television and computer.
Two parents flats and a parent's pantry are available for parents to stay with their children.
Visiting Hours: Parents/caregivers may visit at any time. All other visitors are welcome from 11.00 am - 1.00 pm & 2.30 pm - 8.00 pm, daily.

This service provides telephone counselling, and referral and operates 5.30 pm - 9.00 am, Monday - Friday and 24 hours on weekends and public holidays.
Sexual assault is any behaviour of a sexual nature, which is unwelcome and makes the recipient feel afraid or intimidated. This includes verbal abuse, exhibitionism, fondling, rape and incest.
Ballarat Centre Against Sexual Assault provides free and confidential counselling to sexual assault victims and survivors, whether the assault occurred recently or in the past. Support is also available to family members and significant others. These services are offered to children, and adults, females and males.
Contact with many people may be via telephone, face-to-face or in a group. The needs and wishes of the victims/survivors are the foremost concern and support will be given in line with these needs and wishes.
Outreach services are provided, by appointment, at: Grampians Community Health Centre, Ararat; Djerriwarrh Health Services, Bacchus Marsh & Daylesford Community Health Centre, Daylesford. Appointments should be made through the Ballarat office.

The Perioperative and Day Procedure Unit is a specialised area of Ballarat Health Services, Base Hospital. Care is provided to people who are waiting for either a same day surgical procedure or endoscopy.
The patient comes to this Unit to be admitted and prepared for surgery or endoscopy. In this Unit, they are able to wait (dressed in their own clothes) in comfortable chairs, until it is time for them to go to Theatre. They are then changed into a gown and transferred to Theatre.
The Perioperative and Day Procedure staff are also involved in the assessment of patient health status and the commencement of discharge planning, prior to the patient coming in for surgery or endoscopy. This is carried out through pre-admission clinics and patient health questionnaires.
For some types of surgery or procedure, pre-admission appointments are made automatically by the Booking Office. Perioperative and Day Procedure staff may also arrange pre-admission appointments, after reviewing patient health questionnaires.
Admission to the Perioperative and Day Procedure Unit on the day of surgery is standard practice at Ballarat Health Services - Base Hospital.

Dialysis Service- Provision of renal replacement therapy in the form of haemodialysis, involving the use of a machine to pump blood through an extracorporeal circuit and a filter. This treatment may be permanent or temporary, depending on the patient's suitability for transplantation, or the option of peritoneal dialysis.
The dialysis unit is currently located at 17 Windemere Street North, Ballarat, Vic. Hours of operation are 7.00 am - 10.00 pm on Tuesdays and from 7.00 am - 7.30 pm on all other days, except Sunday, when the unit is closed.
There is parking at the rear of the dialysis unit (either with parking card access or push the button on boom gate for assistance (holiday patients).
Patients can be referred to the unit by a nephrologist or another dialysis unit. If a patient is on holiday or is transferring from another dialysis unit, completion of forms is required.

The Carer Respite Centre provides a single contact point for carers to access respite care and information on other available resources to support them in their caring role.
A Carer is a person who is looking after someone who is frail, aged, has a disability, chronic illness or mental illness, and the Carer can be family, close friend or neighbours.
Respite care enables carers to have a break from their carers role to allow them time to attend appointments, go shopping, social engagements or just some 'time out'.
Care is available as short-term, occasional or emergency respite care.

Delivery of Acute Medical care at home for patients registered as an inpatient of Ballarat Health Services.
Direct care is delivered to patients within a 10 km radius of Ballarat Health Services. For patients who live outside this radius, care is delivered by agreement with the District Nursing Service.
The service is available 7.00 am - 9.00 pm, seven days per week.
Care of the patient is shared between the relevant hospital treatment unit, the patient's general practitioner, hospital nursing staff and district nursing staff. A written medical referral is required to receive treatment through the service. Patients from other public hospitals can be accepted into Hospital in the Home on their return to Ballarat.

When families book in to have their baby at Ballarat Health Services they are provided with the times and dates of ante natal classes. Bookings can be made through the Outpatients Department and classes are held 5th Floor, Ballarat Base Hospital, Drummond St North, Ballarat.

Ballarat Health Services has a number of nurses with expertise in breast care nursing and has a designated breast care nurse position.
In most cases, women being admitted for surgery for breast cancer will be seen by the Breast Care Nurse prior to admission or pre-operative.
Support and information in relation to breast cancer, treatments and other related concerns is provided by the Breast Care Nurse during a persons inpatient stay and post operative.
Longer term follow up care and support is also provided by the Breast Care Nurse of alternative support arrangements made with the woman according to her support requirements.
The Breast Care Nurse can provide information and assistance in obtaining temporary and permanent breast prostheses.

This unit offers chemotherapy and related services to patients including inpatient, day patient and outpatient care.
The service includes:

-- Oncology Nurses
Country patients receiving treatment can access a list of accommodation options from the Social Worker on 03 5320 4790.
Patients may be eligible for government assistance with transport costs if they reside more than one hundred kilometres from the hospital. The Social Worker can assist with claim forms.
