Additional Information: The DHS Disability Access and Response Service provides information about supports and services for people with disabilities, their families and carers. It can provide assistance with planning and support to people with disabilities, their families and carers in their local area. Disability Services also provides a range of services that support people with disabilities and their families including: 1) Disability Client Services teams located in regional offices across Victoria. People can access information, assessment for eligibility and DSA target group, referrals to other disability and generic services and short and longer-term case management support 2) Case Management Services that make sure people who have a disability have the right help to get the support they need 3) Criminal Justice support for people with an intellectual disability who break the law and come into contact with the Police, courts and correctional services 4) Specialist Services Teams that provide assessment, consultation, intervention and training to clients, families and agencies where a client is displaying challenging behaviours 5) Disability Accommodation Services that provide various levels of support and assistance for people with disabilities who are living at home or in other accommodation in the community 6) Respite Services that offer families a short-term break from caring for a family member with a disability.

Additional Information: The Multiple and Complex Needs (MACN) Initiative is underpinned by the Human Services (Complex Needs) Act 2003. The Act was established to ensure that necessary planning for service delivery would occur for some of Victoria's most vulnerable community members. An eligible person for consideration by the MACN Panel is a person who:(a) has attained 16 years of age; and (b) appears to satisfy two or more of the following criteria: (1) Has a mental disorder within the meaning of the Mental Health Act 1986; (2) Has an acquired brain injury; (3) Has an intellectual impairment; (4) Is a drug or alcohol dependent person within the meaning of the Alcoholic and Drug-dependent Persons Act 1968, and (c) has exhibited violent or dangerous behaviour (further criteria must be met); and (d) is in need of intensive supervision and support (further criteria must be met).

Additional Information: A State Government Statutory Child Protectin Program which receives and responds to notifications of alleged child abuse and neglect of children aged 0 - 17 years. Any member of the community is able to make a notification of child abuse or neglect which will be investigated by the Child Protection Program. Child Protection is available at Department of Human Services offices in Ballarat and Horsham with outreach services to other areas, as required.

Additional Information: The School Nursing Program primarily services the families and children in the 6 - 12 years age group. The key function of this program is health surveillance and assessment of school entrants, referral and follow-up services, early detection and intervention on identified health problems and general health promotion, education and support to families and school communities. Regional teams of nurses with expertise in child health and development deliver the programs to State, Catholic and Registered Primary Schools within the Grampians Region. The School Nursing Program also includes a Secondary School Nurse Program that has a primary focus on health promotion and education to address the health needs of the secondary school age population and secondary schools communities in 15 selected State Secondary Colleges within the Grampians region. The School Nursing Program is available at Department of Human Services offices in Ballarat, Ararat and Horsham and outreach services, as required.
