We are a short-term, goal-based service that supports people in the community to meet their rehabilitation goals through an individualised therapy program. Initial assessment will be conducted to discuss your needs and goals. Together we will establish a therapy plan that will best address your needs.
Ask your GP, specialist, or health practitioner to organise a referral for this service.
