Care Coordinators are Registered Nurses who support the management of a client's long-term condition by:
- finding care providers
- accessing specialists

- identifying needs for further assistance by a health professional
- connecting clients to community programs

Cohealth Fitzroy also offer the Cafe Meals Program (CMP) for residents. This program aims to provide increased access to safe, nutritious, affordable meals for people who are isolated, homeless or at risk of homelessness in the City of Yarra. The program does this by providing a social dining experience for otherwise socially isolated people, at cafes that provide nutritious meals at affordable prices.
Eligibility for Cafe Meals Program:
People experiencing or are at risk of homelessness or those unable to prepare meals for themselves and are not accessing prepared meals elsewhere.

Come to drop in clinic or make a Monday afternoon appointment.
