The Cancer Resource Nurse can be contacted directly by people with a cancer diagnosis, family members and care givers. Health Care Professionals are also able to refer clients to the Cancer Resource Nurse.
A Cancer Resource Nurse is a registered nurse who has undertaken additional education in supportive cancer care and specialises in supporting people affected by cancer in their local community.
A Cancer Resource Nurse can:
• Support those affected by cancer at diagnosis, during and after active treatment in navigating the individual's health and emotional needs.
• Arrange referrals between services to assist those affected by cancer manage their diagnosis and treatment.
• Provide information and contacts for cancer services: locally, regionally and state-wide as necessary.
• Communicate on behalf of clients with their Doctors and other health care providers to ensure their health needs are met.
• Facilitate cancer education for patients, community groups and health professionals

The Centre operates 8.30-4.30pm Monday to Friday. This is a service which enables people over the age of 65 who may or may not have disabilities, and/ or special need, to increase their capacity to participate in life's opportunities.
Providing a comfortable place individuals of the community that are socially isolated to go to; Day centre provides outings and activities and a place to meet.
Participants are able to access health and welfare services while attending the Centre including physiotherapy, podiatry, social work, dietitian, speech therapy, occupational therapy, diabetes education and transport. Group meals are also provided for clients. A men's and Ladies getaway group meets monthly- please contact for further details. Areas services Nhill & surrounding districts.

Post Acute Care is a time limited short term intervention designed to assist patients to recuperate following an acute or subacute hospital admission and to facilitate their independence, or transition to continuing care when required.
This care may include, for example, home help, personal care, delivered meals, respite care and home nursing.
The Case Manager will work closely with the client and/or carer and health care workers to ensure that the most appropriate Post Acute Care is provided to those clients with identified needs.
All clients referred and accepted into the program will have:

-- Needs identification
-- Post acute care plan prepared
-- Appropriate services negotiated.
-- Care services monitoring.
The care needs of the client will be reassessed at the end of the allocated time on the program and be discharged or referred to the appropriate care.

To receive additional post acute care services through the PAC program, clients must:
-- Be a patient in a public hospital, either acute or subacute, in Victoria.
-- Be assessed as requiring additional services to assist with recuperation or transition to continuing care following an acute episode
-- Consent to receive additional post acute care services under the PAC program: and
-- Agree to the forwarding of individual client data to the Department for the purposes of program monitoring and evaluation.
Referral process: By discharging hospital, client and/or carer, and any health professional.
Aims
The program aims to achieve a smooth transition for those clients with more complex health needs, returning back into the community from an acute episode in a public hospital, either acute or subacute facility in Victoria. This will be achieved through the provision of coordinated, appropriate and timely Post Acute Care services.
Program objectives:
-- Provide additional home-based services to assist individuals recuperate after hospitalisation;
-- Improve continuity of care for patients following their discharge from hospital;
-- Work collaboratively with hospitals to provide support to patients as an alternative to inpatient admission from the emergency department, where appropriate; and
-- Assist hospitals and subacute services to maximise use of beds for patients who require hospital services<

The Grampians Aged Care Assessment Service - Ballarat Health Services (Grampians ACAS) is a team of health professionals with expertise in health care and community services for older people. The team draws upon the skills of Geriatricians, Community Nurses, Social Workers and a range of allied health professionals including Physiotherapists, Occupational Therapists and Speech Pathologists.
The aim of the Grampians ACAS is to ensure that older people have access to the most appropriate health, community, and/or residential services to meet their needs, and those of carers.
On receiving a referral, Grampians ACAS staff visit older people, preferrably in their own home, to discuss needs and identify the range of options to meet those needs. Grampians ACAS staff can provide information and referral advice and will work closely with individuals and their GP/Service Providers to ensure the most appropriate and helpful range of services available are put into place.
The Aged Care Assessment Service is the only agency authorised by the Commonwealth to approve older people for the Commonwealth funded residential care, residential respite, Community Aged Care Packages and Flexible Care (Extended Aged Care at Home Packages).
Grampians ACAS also has a role to play in identifying service deficits and working with relevant organisations and government to develop services.
