The Adult Day Activity & Support Service offers a welcoming, supportive environment, helping frail, socially isolated older people, and people with dementia and/or disabilities to maintain personal independence and provide support for their carers.
The program offers centre-based activities for adults with activities including craft work, games, music, cooking, information talks, outings, luncheons, visits and special celebrations centring on national or international events.
Day Centre is held 9.00 am - 4.00 pm, Monday - Friday and 4.00 pm - 7.30 pm, Thursday evenings in the Community Rehabilitation Centre. Transport to and from the centre can be arranged.
Fees
$6.50 per session with additional costs for special outings. Fees may be negotiable, if in financial difficulty.

The Diabetes Educator is part of the diabetes team who care for people with diabetes. This team consists of Specialists, General Practitioner, Diabetes Educator, Dietitian, Podiatrist, & Social Worker.
Each member of the team is dedicated to giving comprehensive care to people with diabetes.
The practice of diabetes incudes clinical care, education, advocacy, health promotion and counselling. Through education and support, people are able to obtain the skills to adequately self-manage their condition thereby improving their quality of life.

Cardiac Rehabilitation is the combined use of medical, social, educational, vocational and physical measures to assist people with heart disease to return to an active, satisfying lifestyle.
Each participant is encouraged to attend for a 2 hour session for 8 weeks. During this time the patient will complete the education series and progress through an exercise program at their individual pace. The Pharmacist will also attend regularly to discuss your medications with you individually. Partners are encouraged to attend. Patients are encouraged to continue with the lifestyle changes they have made during cardiac rehabilitation and a number of special activities are run throughout the year which participants are invited to attend including: Learning to do CPR,Healthy Hearts Luncheon,Heart Week Walk,Healthy Cooking Demonstration,Christmas Party

The Wound Clinic provides a nursing service that specialises in wound care management. Leg ulcers fall into different categories and the treatment between each varies considerably. An accurate diagnosis of the cause of each ulcer enables the appropriate treatment to be commenced.
The District Nursing Wound Clinic provides the following services:
Diagnosis of venous and arterial leg ulcers and initiation of appropriate treatment.
Treatment of acute and chronic wounds.
Specialised compression bandaging used for the treatment of venous leg ulcers.
Fitting of compression socks and stockings for the control of swelling in the lower legs, treatment of varicose veins and leg ulcers and prior to flying to prevent the incidence of deep vein thrombosis.

Oxley ward is a 34 bed ward in which acute medical and nursing care is provided to patients with a diverse range of illnesses including minor and major surgery.
Included in the ward in an intensive care unit for specialist treatment of critically ill, high dependency and coronary care patients.
The five intensive care beds ensure close monitoring and highly skilled treatment for patients with complex nursing and medical needs following trauma, major surgery and acute cardiac and respiratory events.
The other 29 beds service a mix of medical and surgical patients.
Two general physicians, in consultation with general practitioners, oversee the care of patients with medical conditions such as acute cardiac, neurological and respiratory conditions, as well as those with palliative care needs.
Two local surgeons and several visiting specialist surgeons provide abdominal, orthopaedic, thoracic, vascular and urology surgery as well as the insertion of cardiac pacemakers.
The Oxley nursing staff provide high quality care to these and other patients.
Additional services available through Oxley Ward include: Pacemaker Clinic, Inpatient Rehabilitation, Stress (exercise) Testing Clinic, Cardiac Rehabilitation and Chemotherapy.

The Rehabilitation Service offers a multi-disciplinary service which includes appraisal and treatment/care from physiotherapists, occupational therapists, dietitians, continence consultants, medical and nursing staff.
A special feature of the Service Unit include a kitchen and dining activities, where patients with disabilities can be helped in re-learning daily activities. Program includes assessment and advice on modification to the home situation as needed.

The Oncology Nurse Practitioner (ONP) can be contacted directly by people with a cancer diagnosis, family members and care givers. Health Care Professionals are also able to refer clients to the Oncology Nurse Practitioner.

An Oncology Nurse Practitioner is a registered nurse educated and authorised to function in an advanced and extended clinical role, in the specialist field of Cancer.
The ONP provides a consultative service which may include:
1. Regular review of patients with complex needs
2. Initial patient consultations with VMO
3. Rapid response consultation; provision of rapid, timely assessment for symptom management and oncological emergencies. Liaison with Medical Oncologist, GP, Emergency staff &/ or Regional Staff.
4. Support and advice on the care and management of Central Venous Access Device (Ports and PICC's)
5 Counselling of newly diagnosed patients
6. Decision making support (treatment /end of life)
Criteria for referral: A
• Patients of WHCG, SRH &/or EGHC diagnosed with cancer and
Criteria B:
- Patients with complex needs which may include one or more of the following:
- Significant physical symptoms of disease or treatment side effects
- Psycho /social issues
- Carer difficulties
- Use of more than three community service providers
Availability: the ONP is available Monday to Friday from 8-4:30, appointments and consultations out of these hours may be arranged

The Day Procedure Unit is part of the operating suite and is designed to allow patients to undergo investigations and procedures without overnight admission to the hospital.
Patients are admitted to the Unit a short time before their surgery or investigation. Overall length of stay is dependent of the type of procedure and anaesthetic. It is usually no more than 3 or 4 hours.

Wax Bath Program:
A program of up to 18 weeks treatment for people with chilblains is held over winter (May - September) each year. The program aims to: -- Increase peripheral blood flow to the feet -- To treat or prevent chilblains. -- To encourage a sedative effect - therefore decrease pain and spasm. -- To assist in maintaining skin integrity. Prosthetics and Orthotics provides assessment, prescription, fabrication, fitting, review and adjustment of prostheses and orthoses as well as ongoing advice and care. Facial, digital and breast prostheses are not supplied by this service.
The service is available to children and adults who:
-- May benefit from orthoses (braces and splints) due to reduced function resulting from musculoskeletal problems, pain, neurological conditions or systematic diseases.
-- Require prostheses (artificial limbs) for functional and cosmetic applications.Ballarat Health Services provides a visiting service, by appointment, on the 2nd & 4th Tuesdays of the month. Appointments should be made by contacting Ballarat Health Services, Ph: 03 5320 3885. New assessments are usually around one hour duration.
Fees
Prostheses are generally funded by the Transport Accident Commission (TAC), WorkCover insurers (WKC), the Department of Veteran's Affairs (DVA) and Ballarat Health Services.
Orthotic clients as per allied health fees policy for consultation. Orthotic clients may also be required to pay for their orthosis. Some may be eligible for assistance via TAC, WKC, DVA,BHS or the Aids and Equipment Program (A&EP). 25 per cent of total orthosis cost for Community Rehabilitation clients.

The Gait and Balance Program utilises a combination of medical, social, educational and physical measures to assist in the prevention of falls for people with walking and and balance disturbances.
Each participant is encouraged to attend weekly sessions from 2.00 pm - 4.00 pm, Fridays, for six weeks. During this time a series of educational topics will be delivered and exercise programs will be tailored and progressed for each individual's needs and abilities.
The program aims to give partipants:
To maintain independence and have confidence in their lifestyle and current place of residence.
Lessen the risk of falling by reducing risk factors through a variety of medical checks, education and graduated exercise.
Assist participants to achieve and maintain an appropriate level of physical activity.

Post Acute Care is a time limited short term intervention designed to assist patients to recuperate following an acute or subacute hospital admission and to facilitate their independence, or transition to continuing care when required.
This care may include, for example, home help, personal care, delivered meals, respite care and home nursing.
The Case Manager will work closely with the client and/or carer and health care workers to ensure that the most appropriate Post Acute Care is provided to those clients with identified needs.
All clients referred and accepted into the program will have:

The Residential In Reach Program (RIR) is part of the Wimmera Health Care Group's Primary Care Program, and run by the District Nursing Department.
The program was launched in February of 2014, with the current catchment area being limited to all Horsham RACFs and other Wimmera areas including Natimuk and Dimboola.
The primary aim of the program is to prevent unnecessary presentations to the Emergency Department of acutely unwell aged care residents by provide education and support for RACF staff in caring for acutely unwell aged care residents and residents with complex needs.
The RIR program is currently free for RACFs to access, including any pharmaceuticals that are short term and directly related to treating the acute illness, and any relevant consumables associated with the acute illness.
RIR support is available Monday to Friday (8am-4.30pm), with after hours coverage for pre-existing clients provided by the Hospital In the Home Service. New referrals are able to be accepted afterhours, but no direct consultation or assessment is available.
Residential Aged Care Residents are able to be referred to the RIR service by:

Wimmera Centre Against Sexual Assault provides free and confidential counselling to sexual assault victims and survivors, whether the assault occurred recently or in the past. Support is also available to family members and significant others. These services are offered to children, and adults, females and males.
Contact with many people may be via telephone, face-to-face or in a group. The needs and wishes of the victims/surviviors are the foremost concern and support will be given in line with these needs and wishes.

BreastScreen provides free mammograms (breast x-rays) to women 40 years of age and over. The early detection of breast cancer offers a better chance of treatment and recovery.
Mammography is a low dose x-ray, whereby each breast is compressed firmly for a few seconds. The x-rays are then read by two radiologists trained in the detection of early breast cancer. Results are forwarded to the client, by letter, within two weeks.
If any changes are detected on the x-ray, BreastScreen offers a free, high quality assessment at St John of God Hospital, Ballarat. This involves counselling and further tests. If cancer is detected, women are referred back to their own doctor for treatment.
Women 50 and over who have no breast symptoms and have not had breast cancer in the pas, are encouraged to attend BreastScreen every two years. Women 40 - 49 have a lower risk of breast cancer, but may join the program if they wish.
Wimmera BreastScreen is part of the Central Highlands and Wimmera BreastScreening & Assessment Service.

A Living at Home Assessment (LAHA) is the process of undertaking a broad holistic assessment of general health and wellbeing in the context of active ageing and independent living.

A LAHA may include service-specific assessment(s) for service(s) provided by the assessing organisation, risk management and occupational health and safety assessment (OHS).

HARP CDM provides a service for people who are identifed as frequent hospital attendees who are most likely to benefit from integrated care and have a chronic disease process.
The key target groups includes:
People with chronic heart disease;
People with chronic respiratory disease;
People with Diabetes
Older people with complex needs; and
People with complex psychosocial needs.
Why these groups of people? Evidence shows that these groups are 'people who frequently use hospitals or who are at risk of hospitalisation'
The HARP-CDM is a client centred service provided through self management principles and the building of individual and community capacity to support a person with a chronic disease process.

Wimmera Community Options provides Linkages as part of a range of services to assist people who are frail aged and/or people with disabilities and their carers.
Linkages is a program for frail older people and younger people with disabilities who are living at home.
Carers are also eligible for assistance. An assessment of each persons needs is required to determine eligibility. Linkages provides case management, coordination and provision of additional services, if required.

Provides case management, education and counselling for patients and their families. The service is suitable for people aged 6 - 65 years who are experiencing difficulties returning to family, work and other roles rollowing brain injury (including stroke).
People with mild ABI are contacted by the Clinic 2 - 3 weeks after the incident to screen for any ongoing issues. The ABI Clinic involvement may last from one initial screening phone call, to ongoing complex case management for more seriously brain injured clients.

Chronic bronchitis and emphysema are common long-term lung diseases that cause shortness of breath. Each condition can occur on its own, but many people have a mixture of the two problems. Chronic Obstructive Airways Disease (COAD) is often used to refer to the condition.
The Pulmonary Rehabilitation Program aims to:
Improve the quality of patients lives
Increase their exercise tolerance
Decrease the number of admissions to hospital
Assist relatives and carers to understand the disease
To provide education and advice to help manage chronic lung disease
The Pulmonary Rehabilitation Program is held weekly for 8 weeks from 2.00 pm - 4.00 pm, Thursdays in the Rehabilitation Gym in the Arapiles Building. The program may include the servics of a Physiotherapist, Occupational Therapist, Dietitian, Pharmacist, Social Worker and Nurse.
Each session consists of exercise, as tolerated, coffee break and an education session. Patients are monitored following completion of the program, four months and twelve months at the completion of the program.
The Pulmonary Rehabilitation Program is located in the Arapiles Building, Baillie St, Horsham.

The Pre-Admission Clinic is designed to allow continuity of integrated care for elective surgical patients requiring admission to the hospital.
The Clinic enables patients to undergo nursing, medical and paramedical assessment and any necessary tests prior to their admission, thus streamlining admission on the day of surgery.
Opportunity is provided for patients to discuss their concerns or special requirements they may have about their operation and hospital stay.
Patient education on pre and post operative care and pain management is discussed, and information is given to patients on the admission procedure and anticipated time in hospital.
