A Living at Home Assessment is the process of undertaking a broad holistic assessment of general health and wellbeing in the context of active ageing and independent living. This assessment is funded by the Department of Health and Human Services (DHHS) in Victoria. The assessment can be requested by clients/family/other agencies (with client consent) and focusses on the needs and goals of the client and works with them to access appropriate community or primary care services.

Regional Assessment Service (RAS) - Commonwealth Home Support Programme (CHSP for people over the age of 65 years or 50 years if the client is an Aboriginal or Torres Strait Islander)
RAS also assess for Assistance with Care and Housing programs.
The RAS home support assessment officer role is to work with clients and their families/carers to develop a support plan that reflects the clients aged care needs, goals and preferences.

The service includes a combination of hospital and home-based nursing and medical care or it may replace hospitalisation completely.

Additional Information: Hospice or palliative care is the care provided for the person who is dying, as well as the family and friends who are closely involved.

Wimmera Hospice Care works with professional health carers, community support services, volunteers and community members who are providing hospice care in their homes, in hospitals and nursing homes in all areas throughout the Wimmera region.

The program provides consultative and supportive service, which includes working with patients and their families as they learn about what kind of assistance they may require during the course of the illness, and liaise with other health professionals and community care providers who may be involved with the patient and family.

The service loans equipment which is used to promote the comfort for the patient and to make care easier for the carer.

The Coordinator is also available to be involved in educational sessions for professionals as well as community groups.

Aims: The program aims to achieve a smooth transition for those clients with more complex health needs, returning back into the community from an acute episode in a public hospital, either acute or subacute facility in Victoria. This will be achieved through the provision of coordinated, appropriate and timely Post Acute Care services.
Program objectives:
- Provide additional home-based services to assist individuals recuperate after hospitalisation;
- Improve continuity of care for patients following their discharge from hospital;
- Work collaboratively with hospitals to provide support to patients as an alternative to inpatient admission from the emergency department, where appropriate; and
- Assist hospitals and subacute services to maximise use of beds for patients who require hospital services.
