Hospice or palliative care is the care provided for the person who is dying, as well as the family and friends who are closely involved.
Wimmera Hospice Care works with professional health carers, community support services, volunteers and community members who are providing hospice care in their homes, in hospitals and nursing homes in all areas throughout the Wimmera region.
The program provides consultative and supportive service, which includes working with patients and their families as they learn about what kind of assistance they may require during the course of the illness, and liaise with other health professionals and community care providers who may be involved with the patient and family.
The service loans equipment which is used to promote the comfort for the patient and to make care easier for the carer.
The Coordinator is also available to be involved in educational sessions for professionals as well as community groups.

No fees apply, although donations are welcome and tax deductible.

Eligibility
The service is available to any terminally ill person and their families. Referral may be made by self, medical, nursing or hospital staff, patients GP needs to be aware of the referral.
