Lancefield Neighbourhood House Inc

Cobaw Club programs are varied recreational and social support group activity programs that operate during the day from Monday to Friday, and some Saturdays. There are some groups based at our Activity Centre at Mollison Place, and some outings programs. Transport is offered to those who have no other way of attending programs. 

The Cobaw Club offers:
-- Appropriate recreational opportunity. 
-- Peer group support and social interaction – making new friends. 
-- Connection with the wider community. 
-- Assistance and support to maintain or improve current level of independence. 
-- Referral to Health professionals if requested 
-- Personal Care assistance if required. 
-- Respite, advice, referral and support for Carers. 
-- Support for specific dietary and/or cultural needs. 
-- Enjoyment and Fun!

Program Fees: $4.00 program fee for each program.  These fees can be waived or reduced if finances are difficult.

Meal Costs: $5.00 per 3 course meal (if eating at Centre).  Outings groups members purchase own meals. Members are welcome to bring their own meal if they wish.

Cobaw Community Health

The Woodend Neighbourhood House was formed after the Ash Wednesday Bush Fires in 1983, to provide a social centre for the victims of the fires.

Since then the House has grown and changed, Today we offer many educational and recreational courses that are open to all members of our Community and surrounding districts. We also continue to offer many social activities and are always open as a drop in centre for those who wish to come and have a cup of coffee and a chat.

The House could not operate without our many wonderful volunteers, some of whom have been with us for many years.  Do come and meet our cheery Volunteers and Staff.

Woodend Neighbourhood House is part of the Macedon Ranges network of Neighbourhood Houses and Community Centres and part of the RANCH Network.

DPV Health - Craigieburn

The program offers friendship and a packed program of stimulating activities each Friday to older people and those with a disability in the Craigieburn area of northern Melbourne.
The activities in this program, auspiced by Hume City Council, include creative arts, music, cultural celebrations, games, interesting guest speakers and regular outings.
We encourage participants to help us plan the activities.
A nutritious three-course lunch and morning and afternoon tea are served.

Brotherhood of St Laurence - Coolibah Social Support Program - Craigieburn

The Planned Activity Group provides a social activities program for eligible frail, aged and younger people with disabilities.  Planned Activity Groups aim to meet the needs and interests of older people in a manner that promotes participation and enjoyment.  
We are located in the Community Services building at the rear of the hospital.

Planned Activity Groups aim to:
Provide programs that encourage participation in a variety of activities that cater for individual interests and capabilities 
Encourage friendships and connectedness within our community 
Meet the needs of people with dementia in a safe and secure environment

Planned Activity Groups provide:
Support with personal care requirements 
Respite and support to carers

Activities include:
Day outings 
Guest speakers 
Music 
Bingo 
Cards 
Exercise and physical games 
Celebration days (Melbourne Cup, Australia Day etc)
We are located in the Community Services Building located at the rear of the hospital.

Hours of Operation 
Our program is centre based with a monthly outing.  The centre is open during weekdays.

Transport:
The Centre has a small bus with wheelchair access and a hoist. The bus is driven by Centre staff to assist you.  Those who can are encouraged to make their own arrangements.

DPV Health - Meadow Heights

Planned Activity Group - High (PAG) provides accessible support services for frail older people and their carers.
PAG provides a comprehensive range of services, programs and activities which give practical and emotional support and social contact for older frail people and older people with disabilities.
Program activities include gentle exercise, discussion groups, outings, personal care, craft and health eduction. PAG is operated in Melton township. Clients attending the centre can be picked up and delivered home by bus. Activities are not limited to the base venue, but also take place in a variety of other community settings.
The day program provides enhancement of daily living skills for the frail aged and those with disabilties. Activities provided include respite for families or carers.
Sessions are available Monday to Friday. Two Saturdays a month carer outings are offered and a carer support group is offered the third Thursday of each month.

A minimal fee applies for this service in line with the HACC fees policy.
