These are:  -- You are a public, Department of Veterans Affairs, Transport Accident Commission or WorkCover patient.  -- You have a carer willing to assist while you are on the program.  -- You live within the catchment area  -- There is a telephone at your nominated residence. - You meet the medical criteria
