Residential Aged Care In-Reach provides hospital type care, where appropriate and safe, to people living in residential aged care facilities (RACF).

Residential Aged Care In-Reach is staffed by Clinical Nurse Consultants and Doctors from the Geriatrician team, who may come to your facility to assess and treat your resident in appropriate circumstances.

Catchment area for this service is City of Melbourne, City of Moreland and City of Yarra.

Yarra City Council Aged Care Services

When a person meets Home and Community Care (HACC) program eligibility criteria an appointment is made with an Assessment Officer for a Living at Home Assessment (LAHA).  A person is visited at home and information is obtained about their abilities, needs and goals.  If required a Care Plan is jointly developed with the person outlining any services to be provided and referrals to be made to achieve the goals of the plan.  Information on Council and other community services available in the municipality is also provided.  No fees apply for this service.

North West ACAS
A multi-disciplinary team of health professionals who assists the elderly and people with disabilities, their families and carers, to make informed decisions about the level of individual care required, whether at home with community support, in a supported residential service, in an aged hostel or a nursing home. The ACAS has specific delegated responsibilities to approve entry to hostels and nursing homes and access to Community Care Packages. Services for aged patients include in-patient care, HACC services, a Community Rehabilitation Center (CRC), a day activities center and medical clinics specialising in issues such as pain, falls and balance, memory and wounds.

ACAS is a team of health professionals who are OT's, Physiotherapists, Social Workers, Community Nurses, and Geriatricians who complete a comprehensive assessment to determine a person's care needs.

Aims: NW West ACAS aims to ensure that frail older people, and people with an age-related disability gain access to services appropriate to their needs. They play a major role in ensuring that services are co-ordinated to the benefit of the client.
Younger people with disabilities are also eligible for assessment services in some circumstances - the Intake Worker will be able to clarify eligibility.
