North West ACAS
A multi-disciplinary team of health professionals who assists the elderly and people with disabilities, their families and carers, to make informed decisions about the level of individual care required, whether at home with community support, in a supported residential service, in an aged hostel or a nursing home. The ACAS has specific delegated responsibilities to approve entry to hostels and nursing homes and access to Community Care Packages. Services for aged patients include in-patient care, HACC services, a Community Rehabilitation Center (CRC), a day activities center and medical clinics specialising in issues such as pain, falls and balance, memory and wounds.

Additional Information
ACAS is a team of health professionals who are OT's, Physiotherapists, Social Workers, Community Nurses, and Geriatricians who complete a comprehensive assessment to determine a person's care needs.
