The EWHS Central Intake is the gateway to all of EWHS community based services. On the intital contact all consumers will speak with a Community Health Nurse, who will assist the consumer to identify the consumers health priorities.

East Wimmera Health Service - Community Health

Service Coordination places consumers at the centre of service delivery to make sure they have access to the services they need, opportunities for early intervention and health promotion, and improved health outcomes.

Service Coordination enables health and community services professionals to work together to ensure a coordinated and integrated response to consumer needs.

SMPCP staff are able to support member agencies to implement service coordination practices and e-referral.

IPC Health - St Albans

IPC Health - Deer Park

Facilitates the relationship between Indigenous clients and health care providers; including specialists, general practitioners, psychologists, practice nurses and allied health professionals. This includes:  Accepting referrals to assist GPs to manage patient care. Intensive assistance related to chronic disease management. Helping clients to schedule and attend appointments, accompanying them when appropriate.

An information and referral service for community members and service providers seeking health information and services. Information and advice on contraception, sexual health, menopause, pregnancy and any other women's health issues.
