IPC Health - St Albans

Ensures the safe and effective discharge of ED patients who are medically cleared for discharge, but are screened to be at risk of not managing in the community.  Needs assessment includes: • Whether the patient lives alone • Whether they are likely to have self care problems • If they care for other people • If they already using community services  An action plan is then implemented to address each of these issues in a timely and appropriate manner in order to ensure a safe patient outcome.

Works to stop the stigma related to hepatitis by striving to educate the community about the virus and to empower people with hepatitis to become self-advocates and to redress any discrimination they may have experienced, including working with them for the duration of any legal cases. Badjurr-Bulok Wilam, Aboriginal and Torres Strait Islander Hospital Liaison Officers (ATSIHLO)- Badjurr-Bulok Wilam, meaning 'Home of many women' in the Woiwurrung language of the Wurundjeri Peoples, provides culturally appropriate support and advocacy services to Aboriginal and Torres Strait Islander women to address social, emotional and cultural needs. Services offered include accessing the hospital's services, information and referrals to services and practical assistance. Badjurr-Bulok Wilam provides a resting place for Aboriginal and Torres Strait Islander women and families attending the Women's, where women and families can sit and yarn with the Aboriginal and Torres Strait Islander Hospital Liaison Officers (ATSIHLO). It is best to call and make a time to catch up with staff at Badjurr-Bulok Wilam to avoid having to wait.
