IPC Health - St Albans

Ensures the safe and effective discharge of ED patients who are medically cleared for discharge, but are screened to be at risk of not managing in the community.  Needs assessment includes: • Whether the patient lives alone • Whether they are likely to have self care problems • If they care for other people • If they already using community services  An action plan is then implemented to address each of these issues in a timely and appropriate manner in order to ensure a safe patient outcome.

*Koori Maternity Services (KMS) works with the Midwife to support Aboriginal and Torres Strait Islander women and their families in their pregnancy journey.

Footscray Hospital - Western Health

Wadja Aboriginal Family Place- Wadja Health Clinic is a weekly general medical clinic for Aboriginal children which provides medical, social, cultural and emotional assessment. The focus of the clinic is on the management of Aboriginal patients with complex health and social issues by a team.

The Royal Children's Hospital

Works to stop the stigma related to hepatitis by striving to educate the community about the virus and to empower people with hepatitis to become self-advocates and to redress any discrimination they may have experienced, including working with them for the duration of any legal cases. Provides training on best practice around hepatitis in relation to infection control procedures and OH&S guidelines.
