On receiving a referral, Grampians Aged Care Assesment Service (ACAS) staff visit older people, preferably in their own home, to discuss the needs and identify a range of options to meet those needs.
ACAS staff can provide information and referral advice and will work closely with individuals and their GP/Service Providers to ensure the most appropriate and helpful range of services available are put into place.
