Community Hospital Interface Programme (CHIP) plans for the smooth transition of mother and baby from hospital to community based services. During the ante-natal and post-natal period, if the mother gives consent, the CHIP Nurse will complete a risk assessment, interview the mother and plan for the initiation of the most appropriate services available to meet her needs as a parent.
Services that maybe utilised include Community Child Health, Indigenous Child Health Drug and Alcohol Services, Immunisation clinics, Day Stay feeding/sleep and settling clinics, Parents Aide assistance, Young Parent groups, Mental Health Services, Refugee Services, General Practitioners and Child Safety Officers.
