The Community Connection Program at AccessCare Southern endeavours to engage people in the community who are homeless or at risk of homelessness and support them to engage with services and supports available. Anyone can make a referral to the Community Connection Program, the Case Managers who deliver the program also complete outreach in the community to engage with people who are vulnerable.
