Referral Pathway - Referrals are required from a GP. Other Health professionals can make a provisional referral however a named GP is a requirement.
All referrals should be sent to either the Counselling team email on MMEX or via fax on 08 9192 7999.
Referrals should be made on the Boab Mental Health Teams referral form. Please remember to tick the correct box for the service you require and provide clients contact details including a telephone number, PO Box or street address.
Referral Process.
Once a referral is received a letter will be sent to the client and the referrer confirming receipt and approximate wait times.
Once a Mental Health Practitioner becomes available they will contact the client directly to arrange an assessment.
After assessment the Mental Health Practitioner will either offer the client individual counseling, self help materials or signpost the client to a more appropriate service if we are unable to offer assistance.
