An AccessCare Southern Home Care Package is tailored to meet your unique needs or those of your carer through case management and funding for services. The Home Care Package program is funded by the Commonwealth Government for people who are older with complex needs. An Aged Care Assessment Service will determine whether you are eligible for the program and if so which of the four levels of package you require. AccessCare Southern have an allocation to deliver Home Care Packages Level 1, 2 and 4 in the municipalities of Kingston, Bayside, Glen Eira, Port Phillip and Stonnington . Services available through your Home Care Package include home care, personal care (assistance with showering, dressing, cooking or shopping), respite, delivered meals, social support and transport. A Case Manager is assigned as part of every Home Care Package to assess, plan and implement a range of services and supports to meet your individual needs. The Case Managers at AccessCare Southern understand the full range of services and options available. The level of Case Manager involvement can be negotiated. They will form a partnership with you to respectfully share their knowledge to help you to make the best decisions regarding your care. To receive a Home Care Package fees apply.
