Delivery of Acute Medical care at home for patients registered as an inpatient of Ballarat Health Services.
Direct care is delivered to patients within a 10 km radius of Ballarat Health Services. For patients who live outside this radius, care is delivered by agreement with the District Nursing Service.
The service is available 7.00 am - 9.00 pm, seven days per week.
Care of the patient is shared between the relevant hospital treatment unit, the patient's general practitioner, hospital nursing staff and district nursing staff. A written medical referral is required to receive treatment through the service. Patients from other public hospitals can be accepted into Hospital in the Home on their return to Ballarat.
Fees
No fees. The cost of care delivery is covered as an inpatient of Ballarat Health Services.
