Assisted care and housing for the aged.
Assistance provided to low income frail and at risk elderly people to access and be maintained in secure and affordable housing with support, by;
- Negotiating housing needs
- Assistance with documentation
- Acessing appropriate community support services and material aid on behalf of clients.
The ACHA worker is available four days per week and can be contacted on the number below for a referral. (Appointment made with ACHA worker.)

Additional Information:
The Home Based Allied Health Service visits the frail elderly and younger disabled who are housebound and are unable to access other community services.
Services available are:
• Dietetics
• Speech Pathology
• Physiotherapy
• Occupational Therapy
• Podiatry
• Community Health Nurse and Allied Health Assistant
The aim is to provide assessment and therapy to clients in their own home to improve or maintain their level of function. The needs of the carer, if present, is also assessed and a small number of hours of non-brokerage case management is used to assist.
The program runs a Carers group to provide ongoing education and support for the carers of clients with Alzheimer's disease. The Service operates Monday to Friday only.
Cost
HACC fees policy applies. Standard visit cost is $7.00 per visit, Podiatry is $15.00 per visit.
