Carer Support Services provides a 'first contact' information point for carers, to assist and support them in accessing services including respite care.
Who can we help
Carer Support provides a clearly and readily accessible regional focal point for carers providing information and support about the full range of services and other resources available to those in a caring role in the Swan Hill, Gannawarra and Buloke regions.
Carers come from all backgrounds, and range from the very young to the elderly. Carers give their time and energy to those for whom they care. But looking after someone can be exhausting and seem never ending. Some carers have support, but many feel very much alone.
Carer Support Services was established as part of the Victorian Carer Initiatives and the Commonwealth National Respite for Carers Strategy in the Loddon Mallee region. This team provides a 'first contact' information point for carers, to assist and support them in accessing services including respite care.
Commonwealth Carer Respite Centres work with carers to plan sensible approaches to respite and other support needs and also arrange 24 hour emergency respite care.
Our Regional Residential Respite Carer Support Worker is based in the Bendigo Office, and contactable on the above 1800 number.
The service:

-- Provides practical and emotional support to carers
-- Has 24-hour emergency access

Aged Care Assessment Services (ACAS) is a multi-disciplinary team who assist older people and their carers to identify what kind of care will best meet their needs when they are no longer able to manage at home without assistance.
ACAS provides a comprehensive assessment of care needs and information on suitable care options including Community Care Services, Community Aged Care Packages or Residential Aged Care services. They help to arrange access or referral to appropriate care needs.
Bendigo ACAS provides services for the Loddon Mallee region, except Mildura area.
Staff include Geriatricians, Nurses and Allied Health staff.

Referral Process
Bendigo Health Referral Centre accepts refererals from inpatient services, hospital outpatient and ambulatory services, general practitioners, allied health and self referrals. The preferred referral method is via the SCTT form, although Medical Director summary templates from GP's are also accepted.
Basic demographic information is required including full name, date of birth, contact details including address and telephone number, gender, GP details, consultant details if applicable and the medical condition/reason for referral.

The Chronic Illness Relief program's target group is recognised as being very broad.
People with Arthritis, Cystic Fibrosis, Diabetes, Epilepsy, Motor Neurone, Multiple Sclerosis, Muscular Dystrophy, Parkinson's, Thalassemia, allergies, cancer, HIV/AIDS, haemophilia & chemical sensitivity are considered a priority.
People on waiting lists for services and people receiving services but whose needs could best be met by a top up of funding, are also considered a priority.

Flexible Care Packages use case management and discretionary funds to assist and support families and carers to care for people with a disability at home.
The packages, which aim to maximise independence and participation in the community, are designed to prevent the need for more costly services or long-term residential placement.
Flexible Support Packages offer support to individuals seeking short-term assistance, approximately 3 months duration, and people requiring intensive or longer term support.
Case Managers work in partnership with individuals and their families and carers to identify support needs and develop a plan to increase the individual's capacity and family's resilience.
Who Can We Help?
Flexible Support Packages are designed for children, young people and adults with a disability, as well as their families and carers, where relevant.
Packages are allocated according to the relative need of people and consideration of the following:

-- Carers with fragile health
-- People who require brief and timely intervention to prevent or minimise the need for long-term high cost support
-- People who have high or complex support needs

-- Those who are in a situation of high risk or vulnerability.
-- Informal (unpaid) carer relationship is at risk of breaking down, thus leaving the person at risk of institutionalisation.
