Additional Information:
Communities@Work's transport service is a lifeline to many people. It assists clients to have continued independence and confidence by transporting them to medical and para-medical appointments, social and community activities and shopping centres. We also transport clients to visit loved ones who are living in residential care.
We have a pool of experienced, casual drivers who provide physical assistance to clients when needed, assisting the client to and from the vehicle and escorting them to their appointment.
A small group of dedicated volunteers also assist the program by providing transport to medical appointments, assistance with shopping and social appointments; they also provide transport services for clients over the weekend.
HACC Community Transport
If you are HACC eligible and have no other way of getting to medical or social appointments, our experienced staff and volunteers can assist you from door to door.
Qualified drivers from our team can be booked from between 7.00 am and 7.00 pm Monday to Friday. Limited trips are available on weekends.
Community Bus Service
The community minibus service is for seniors and other people who are isolated in their communities through limited transport options. It is available to:
- seniors with mobility or confidence problems;
- people living in residential care/nursing homes;
- people with mental illness who cannot cope with using regular ACTION services;
- people with permanent or temporary disability;
- carers accompanying any of the above;
- parents with young children who are socially isolated through a lack of transport options;
- people with culturally and linguistically diverse backgrounds, including new migrants, whose transport options may be limited due to factors such as their unfamiliarity with transport services and/or lack of support networks; and
- people who are eligible for HACC transport services but cannot access them - for example, people who require transport for 'cross-catchment' trips, or people with a disability who use wheelchairs and cannot self-transfer into vehicle seats.
Qualified drivers from our team can be booked between 7.00 am and 7.00 pm Monday to Friday. Limited trips are available on weekends.

Additional Information: The SuperGrands Program is a free mentoring service for families to help strengthen living skills. Mature, trained volunteers are carefully matched with families to offer one-on-one home mentoring and/or group coaching in areas like budgeting, meal planning, establishing routines and basic home maintenance. SuperGrands is not about 'doing it for you today' but giving you the skills to 'do it for tomorrow'. It's about sharing skills between generations and is ideal for young parents or families without an extended family nearby. SuperGrands is all about strengthening skills that families want to develop. The program involves your family, the SuperGrands Coordinator and the SuperGrands team - all working together. You can refer your own family or ask a support person to do so. For more information on this program please visit our website through the link provided or call one of our friendly staff members.
