During this time you can expect to: * Explore your asthma history * Have your breathing tested (spirometry) * Review your medications * Learn proper usage of medication devices * Explore triggers that make your asthma worse * Develop strategies to deal with triggers * Create a plan to control your asthma (rather than it controlling you). More than one session may be required for people with moderate to severe asthma. A personal asthma plan improves your management of asthma — don't wait until you are sick to treat it. What is an exercise physiologist?
An exercise physiologist is a university qualified professional who holds accreditation with the Australian Association of Exercise & Sports Science.
Exercise physiologists specialise in human performance and physiology—the mechanical, physical and biochemical functions of our bodies.
Our exercise physiologist offers individualised care to meet your personal health needs. She is a sensitive and caring person who understands the many difficulties people face with exercise. Our Exercise Physiologist works with you to develop a plan that works at your pace and meets your needs.
Wear comfortable loose clothing & sports or flat heeled shoes and bring a water bottle.
In partnership with you, and your GP, our exercise physiologist works to consider your whole health needs when creating an exercise plan that is right for you.

The Brooke St. Medical Centre Dietitian's key role is to assess your personal needs and advise you on an individualised eating plan. This plan will meet your nutrition, weight and health needs. If you a have particular condition, the plan will be tailored to address this. Our Dietitian can work with: * Weight loss * Healthy eating plans * Advice on special needs * Food allergy & intolerances * Improved energy * Sustenance & nutrition * Chronic disease prevention & management. Healthy diet improves your general health and wellbeing. Our Dietitian can help you avoid or manage chronic disease with a plan tailored to your needs. The Brooke St. Medical Centre Dietitian offers individualised care to meet your personal health needs. Our Dietitian is a caring person who understands the many difficulties people face with diet. She will work with you to develop a flexible and sustainable plan that addresses your personal needs. Your whole health needs are our concern. The Brooke St. Medical Centre Dietitian will work in partnership with you, your GP and any other service providers you wish to include to consider all your needs. What can I expect? A longer first session to enable a comprehensive assessment of your needs. This will include taking a diet history. After this you will usually have review sessions with the Dietitian to develop a personal plan and receive ongoing support to manage dietary habits. What does it cost? Our psychologists are experienced health professionals who work with children, teenagers and adults to help with a range of issues including: general worries or stress; grief; depression and anxiety; other mental health problems; relationship issues; parenting etc.
We offer services for patients referred under Medicare Mental Health Care Plans, TAC, WorkSafe, DVA, Victims of Crime, Employee Assistance Programs etc. However, you don't need a referral to make an appointment.

Your Diabetes Team is made up of:
* Your Doctor / GP
* Diabetes Nurse Educator
* And other health care providers such as Dietitian, Exercise Physiologist, and Podiatrist as required. Referrals can be made to Allied Health Practitioners and Specialists to meet your particular needs.
What care can I expect from the Diabetes Team
In order to provide you with the best possible care, you can expect Brooke Street Medical Centre Diabetes Team to do the following, as a minimum:
* Pathology tests
* Foot checks
* Blood Pressure checks
* Body Mass Index readings
* Medication Reviews
* Self-Care education
* Referral for Eye checks
The team is also here to assist you with your understanding of diabetes and managing such things as your home based blood glucose monitoring, diet, etc.
Your GP and Diabetes Educator will conduct a thorough assessment to determine your personal needs and create a care plan with you accordingly.
A Team Care Arrangement may be suggested to ensure the thorough coordination of your care where a number of service providers are involved in your care.
How often should I have checks and tests done?
This depends on your own personal needs, but as a minimum, blood tests should be done 6 monthly to test your diabetes control. Other tests and checks may need to be done less often. Your GP and Diabetes Educator will advise you, according to your individual needs.
Ongoing appointments will be made as a matter of good practice. People who fail to maintain their regular checks are more at risk of developing complications and reduced control of their diabetes.
When can I receive specialist diabetes care?
For your convenience, appointments with the
* The Diabetes Educator are available on Monday, Tuesday, Wednesday and Friday.
* The Dietitian is available Wednesday to Friday and our Exercise Physiologist is available Monday to Friday.
* Your GP is available at various times and will link in with these appointments as required.
How will I know when my tests and checks are due?
Brooke Street Medical Centre is works to coordinate your care within the recommended time frames. You may therefore be contacted by one of our team when our records show that you are due for tests or checks. We strive to ensure that your next appointment is made before you leave the practice to promote optimal health care. Of course you can simply call us at any time to make an
appointment and discuss your health needs — phone 5427 1002.
What if I am a patient newly diagnosed with diabetes?
Our Diabetes Educator is available, with your GP, to spend time helping you understand this condition and advise you how to care for yourself. We understand that this can be a stressful time and aim to tailor treatment to meet your personal needs.
