Home and Community Care Program for Younger People (HACC PYP for people under 65 (under 50 if Aboriginal or Torres Strait Islander) – Living at Home Assessments
A Living at Home Assessment is the process of undertaking a broad holistic assessment of general health and wellbeing in the context of active ageing and independent living. This assessment is funded by the Department of Health and Human Services (DHHS) in Victoria. The assessment can be requested by clients/family/other agencies (with client consent) and focusses on the needs and goals of the client and works with them to access appropriate community or primary care services.
Regional Assessment Service (RAS) - Commonwealth Home Support Programme (CHSP for people over the age of 65 years or 50 years if the client is an Aboriginal or Torres Strait Islander)
RAS also assess for Assistance with Care and Housing programs.
The RAS home support assessment officer role is to work with clients and their families/carers to develop a support plan that reflects the clients aged care needs, goals and preferences.
