Bathurst Hospital Respiratory Co-Ordinated Care Program

The Bathurst Diabetes service will provide Diabetes education and clinical support to all people in Bathurst who either live with Diabetes, support others with Diabetes, or who medically manage Diabetes.
An outreach service is provided to Blayney on the 3rd Thursday of every month.
Referrals and appointments required.

'A chronic disease management service for people over the age of 16 years with congestive heart failure, coronary artery disease, chronic obstructive pulmonary disease or diabetes who
