Peninsula Health - Frankston Community Health

Peninsula Health Community Health provides a broad range of affordable services for people living in Frankston and on the Mornington Peninsula. These services aim to keep people active and healthy as well as provide treatment and advice to help people regain their health. Any person of any age can access services. You can contact us yourself or be referred by your GP, Health Worker or other service. Priority is given to those with special needs, low income and chronic health conditions. Eligibility criteria and small fees apply for most of our services. Dental services are available to Health Care Card Holders and others who meet the assessment criteria. Difficulty or inability to pay will not be a barrier to receiving treatment or services. For a dental appointment phone (03)9784 8184. EACH - Ringwood East

The Aboriginal Care Coordination program is targeted to Aboriginal & Torres Strait Islander people who have a chronic health condition.

Care Coordinators are Registered Nurses who support the management of a client's long-term condition by:
- finding care providers
- accessing specialists
- helping clients understand the importance of their treatment plan and medications
- identifying needs for further assistance by a health professional
- connecting clients to community programs

Eligibility: Any Aboriginal and Torres Strait Islander person with a diagnosed long-term (at least 6 months) health condition.

Cohealth - Fitzroy

Additional Info: Child care is available at limited times while parents are using services at the centre.
With a telephone answering machine available when the office is unattended.

The University of Melbourne - Health Service

The Aboriginal Health Service provides support to patients and families with Aboriginal and/or Torres Strait Islander Peoples backgrounds. It also helps provide access to community organisations. An Aboriginal Service Development Worker works alongside hospital staff to help improve the care and support plans for Aboriginal and/or Torres Strait Islander Peoples patients while in hospital. Aboriginal Health is responsible for: 1. Delivery of culturally responsive services for all Aboriginal and Torres Strait Islander patients and their families when coming to the hospital. 2. Availability of appropriate health services and preventative care for Aboriginal people. 3. Helping ensure the hospital meets the particular needs of Aboriginal and/or Torres Strait Islander Peoples patients and their families and increase the cultural sensitivity of healthcare services and providers to Aboriginal health issues. 4. Build positive relationships with the Victorian Aboriginal community, other service providers, and government departments. Referral- Patients do not need a referral to access this service.
