The Rural and Regional Project is about building an integrated regional support network across Victoria for people living with CF. This is to be achieved by aligning local government, local hospital, Community Health Services, and other relevant local providers, into a regional network - then linking each of the regional networks to one or more of the CF Specialist Centres in Melbourne.
The Project also involves educating and training relevant Health Professionals (i.e. Physiotherapists) to monitor and deliver care at a local level. This may assist in maintaining health levels and reduce the need for untimely visits to Melbourne.
Objectives of the Project;
-- Development of an integrated health and well-being services for people living with CF in rural and regional areas.
-- Assess needs - focus on the individual living with CF and their family/ friends – and assess gaps in services / supports.
-- De-centralised model of care for CF.
-- Improved CF awareness in Rural and Regional Areas.
-- Provision of CF education and training for health and community care workers.
-- Improved access to local and timely community support services.
-- Reduced need to travel to CF Centres – if baseline health is maintained.
-- Improved linkages between local service providers and between regional networks and CF Centres.
-- Reduced household, community and healthcare system costs; and
-- Improved support and treatment adherence and hence greater longevity and quality of life for those living with CF.
