Virtual care to provide early assessment and intervention to suitable patients within the MNCLHD with a focus on those at at risk of presentation to hospital, care navigation, and health education. Through a comprehensive pathway utilising video and audio calls, care encompasses Nursing triage, Medical Assessment with one of our GPs, symptom monitoring.
