OT service includes:
•Assessment and treatment to maximise independence
•Prescription of assistive equipment eg. bath seats, wheelchairs etc.
•Minor and major home modifications
•Adapting activities of daily living
•Health promotion and education
Referral requirements>
•Ensure client is set up for safe home discharge
•Ensure you to contact HBAH OT for handover of information for outstanding SWEP
