Additional information
The Mobile GP project provides healthcare to people aged 50+ with a chronic condition who are homeless or at risk and don’t have a regular GP. Services are offered at Ozanam House in North Melbourne via outreach and telehealth. Includes GP, geriatrician, physio, and mental health support. Hospitals are encouraged to refer patients after discharge if they lack a regular GP or can’t afford private care.
