The HOMHS initiative is aimed at providing adults who experience severe and enduring mental illness and a history of repeated homelessness, a pathway to better health, secure housing and increased social and economic tenure.
HOMHS offers:
• Intensive case management (up to 6 hours per week per consumer)
• Assertive outreach
• Use of an evidence-based recovery oriented model, the Collaborative Recovery Model, which is strengths-based, future-oriented and focussed on meaningful goal setting and attainment; alongside clinical case management
• Care coordination and sustained case management
• Expertise in mental health, homelessness and housing, women’s specific issues and primary health
• A team approach to client care drawing on the partner organisations’ specialist knowledge.
**Eligibility**
• 16-64 years of age
• Experience of a severe and enduring mental disorder with a high degree of clinical complexity, who require ongoing management and support of their illness
• History of long term or repeated homelessness, i.e.: have slept rough continuously for 12 months or more or were homeless episodically for at least three years
• Reside, or have a very strong connection, to the City of Moonee Valley or City of Melbourne
• Or women who meet the above criteria but who may not meet the criteria for primary homelessness and may instead fall into the category of secondary or tertiary homelessness.
