Northern District Community Health Service offers a variety of services to the person with diabetes.
DIABETES SUPPLIES
This scheme provides approved products at subsidized prices (test strips, syringes, needles for insulin pens).
This service is available to all registered members (no membership fee).
To become a registered member, applications forms are available from N.D.C.H.S. and must be completed by both client and doctor.
A membership registration number is necessary to obtain products.
DIABETES SUPPORT
Support and education for individuals and their families with diabetes.
Diabetes Australia Victoria (D.A.V) membership forms are available through Community Health.
Local Diabetes Support Groups meet on a regular basis. Contact N.D.C.H.S. for further details regarding your nearest group.
DIABETES EDUCATION AND HEALTH CARE
Appointments are necessary for the following:
DIABETES EDUCTAION GROUP - Dietician and Diabetes Educator run this group every fortnight for newly diagnosed diabetes, or people needing updates.
DIABETES NURSE EDUCATOR - Provides education regarding all aspects of Diabetes management, including blood glucose testing, insulin, medication, diet and exercise information. This service is available to individuals and groups.
DIETITIAN - Dietary guidelines available for the management of diabetes, high cholesterol, weight management and all other nutrition issues.
PODIATRIST - A subsidized service is available to those over 60 years, disabled or with diabetes in Kerang, Cohuna, Quambatook and Pyramid Hill.
EQUIPMENT PURCHASES AND LOANS
Blood Glucose Meters and Insulin Pens are available through the Diabetes Nurse Educator.
DISPOSAL OF NEEDLES/SYRINGES
Provided through N.D.C.H.S. at cost price.
